RESUMO

Experimental studies of nuclear fission induced by fusion, transfer, spallation, fragmentation, and electromagnetic reactions in combination with state-of-the-art calculations are successful to investigate the nuclear dissipation mechanism in normal nuclear matter, containing only nucleons. The dissipation mechanism has been widely studied by the use of many different fission observables and nowadays the dissipation coefficients involved in transport theories are well constrained. However, the existence of hypernuclei and the possible presence of hyperons in neutron stars make it necessary to extend the investigation of the nuclear dissipation coefficient to the strangeness sector. In this Letter, we use fission reactions of hypernuclei to constrain for the first time the dissipation coefficient in hypernuclear matter, observing that this coefficient increases a factor of 6 in the presence of a single Λ hyperon with respect to normal nuclear matter.

RESUMO

Pleural effusions in children (PE) due to ventricle-peritoneal shunt (VPS) is very rare, with few cases reported. We present a new case of an infant with VPS who had a massive hydrothorax not associated with misplacement or migration of the distal catheter or with ascites. After the evacuation of the PE we managed the patient by adjusting the pressure of the adjustable valve (AV). Sequential thoracic ultrasounds showed a satisfactory outcome. We review the literature thoroughly and describe the possible pathophysiological mechanisms.

Introducción: Las recetas médicas o prescripciones históricas en el territorio de la Nueva Granada (hoy Colombia) revelan la evolución del manejo de la enfermedad en función de los efectos benéficos de administrar algún tipo de agente medicinal de origen vegetal, animal o mineral. Objetivo: Describir el tratamiento del reumatismo en los siglos XVIII y XIX, con base en dos recetas médicas de la época con base en la fundamentación contemporánea de su interacción fisiológica. Materiales y métodos: Búsqueda documental en el archivo Histórico de la Biblioteca Octavio Arizmendi Posada de la Universidad de La Sabana, donde se encontraron las recetas tituladas "Reumatismo" y "Rehumas". Resultados: Se describen cuatro clases de tratamientos para las enfermedades reumáticas utilizados en los siglos XVIII y XIX y orientados principalmente a la disminución del dolor. Este artículo presenta las bases fisiológicas de estos tratamientos, y cómo pudieron o no haber tenido algún efecto calmante al examinar su fisiología. Conclusiones: Estos medicamentos coloniales neogranadinos no ameritan su uso en el contexto actual de la medicina, pero pudieron representar una ayuda terapéutica en su momento.


Introduction: the medical prescriptions or historical prescriptions in the territory of New Granada (present-day Colombia) reveal the evolution of disease management given the beneficial effects of administering some type of medicinal agent of plant, animal or mineral origin. Objective: To describe the treatment of rheumatism in the 18th and 19th centuries based on two medical prescriptions of the time based on the contemporary rationale of their physiological interactions. Materials and methods: Documentary search in the Historical Archive of the Octavio Arizmendi Posada Library at Universidad de La Sabana in Colombia, where the prescriptions entitled "Reumatismo" and "Rehuma" were found. Results: Four classes of treatments for rheumatic diseases used in the 18th and 19th centuries and mainly oriented to pain reduction are described. This article presents the physiological basis of these treatments, and how they may or may not have had some calming effect when examining their physiology. Conclusions: These Neogranadian colonial drugs do not deserve their use in the current context of medicine, but they may have represented a therapeutic aid at the time.

Taking benefit of the R3B/SOFIA setup to measure the mass and the nuclear charge of both fission fragments in coincidence with the total prompt-neutron multiplicity, the scission configurations are inferred along the thorium chain, from the asymmetric fission in the heavier isotopes to the symmetric fission in the neutron-deficient thorium. Against all expectations, the symmetric scission in the light thorium isotopes shows a compact configuration, which is in total contrast to what is known in the fission of the heavier thorium isotopes and heavier actinides. This new main symmetric scission mode is characterized by a significant drop in deformation energy of the fission fragments of about 19 MeV, compared to the well-known symmetric scission in the uranium-plutonium region.

PURPOSE: The aim of this study is to evaluate the incidence of consecutive strabismus after infantile nystagmus surgery and its potential risk factors. METHODS: A retrospective study including 89 patients was conducted. Patients presented infantile nystagmus (idiopathic or ocular disease-associated nystagmus) without previous or coincidental strabismus. Sex, age at surgery, amblyopia, botulinum toxin (BT) injection before surgery, spherical equivalent, anisometropia, surgery procedure (Anderson's or retroequatorial recessions of four horizontal recti), and follow-up were analyzed. Kaplan-Meier and univariate Cox regression were performed. RESULTS: The median age at surgery was 5 years. The median follow-up was 36 months. The incidence of consecutive strabismus was 11.2%. There were eight patients with exotropia and two patients with esotropia. Consecutive strabismus was associated with severe bilateral amblyopia (p = 0.036), previous treatment with BT injection (p = 0.025), and large recessions of the four horizontal muscles (p = 0.001). The hazard ratio for patients with severe bilateral amblyopia was 5.4 (95% CI 1.1-25.8), and for patients previously treated with BT was 6.1 (1.3-29.3). The survival rate was 95.4% at 6 months and 88.5% at 3 years. CONCLUSION: Severe bilateral amblyopia, previous BT treatment, and type of surgery seem to be associated with consecutive strabismus after infantile nystagmus surgery. Most cases appear within the first months after surgery.

Spectroscopic factors of neutron-hole and proton-hole states in ^{131}Sn and ^{131}In, respectively, were measured using one-nucleon removal reactions from doubly magic ^{132}Sn at relativistic energies. For ^{131}In, a 2910(50)-keV γ ray was observed for the first time and tentatively assigned to a decay from a 5/2^{-} state at 3275(50) keV to the known 1/2^{-} level at 365 keV. The spectroscopic factors determined for this new excited state and three other single-hole states provide first evidence for a strong fragmentation of single-hole strength in ^{131}Sn and ^{131}In. The experimental results are compared to theoretical calculations based on the relativistic particle-vibration coupling model and to experimental information for single-hole states in the stable doubly magic nucleus ^{208}Pb.

The aim of this study was to evaluate the effect of a negative energy balance during the first third of gestation on metabolic, endocrine, and pregnancy recognition parameters in 2 beef cattle breeds adapted to semiextensive conditions. Seventy-five lactating Parda de Montaña and 40 Pirenaica multiparous cows rearing calves were synchronized and timed artificial inseminated (TAI) on day 76 postpartum. Cows were assigned to one of 2 diets (CONTROL or SUBNUT; 100% or 65% of their requirements supplied) until day 82 of gestation. Pregnancy was diagnosed 37 d post-TAI using ultrasound. Blood samples were obtained to determine metabolic (glucose, NEFA, ß-hydroxybutyrate, cholesterol, and urea) and endocrine (IGF-1) status throughout the first third of gestation and to determine the concentrations of progesterone and pregnancy-specific protein B (PSPB) in the peri-implantational period. Undernutrition affected both cow and calf performance. The CONTROL cows maintained BCS and BW, whereas SUBNUT cows had negative daily gains. The CONTROL lactating calves had higher BW gains than SUBNUT. These negative effects were more evident in the Pirenaica breed, which was more sensitive to undernutrition. The negative energy balance was reflected in the cows' metabolic profiles, with higher NEFA values and lower IGF-1 concentrations in SUBNUT cows. However, undernutrition did not affect dam pregnancy/TAI or pregnancy recognition and maintenance, confirming that during periods of undernourishment pregnant dams prioritize the allocation of dietary energy toward reproductive functions. Progesterone concentration on day 21 post-TAI (with a 4.8 ng/mL cut-off value) and PSPB on day 26 post-TAI (with a 0.57 ng/mL cut-off value) were determined as the earliest indicators to accurately establish dam pregnancy status, regardless of breed or nutrition treatment. In summary, early undernutrition affected cow performance and metabolic profiles and impaired lactating calf growth, but did not affect progesterone or PSPB concentrations or the pregnancy/TAI rate in suckled cows.

Fifty-five inclusive single nucleon-removal cross sections from medium mass neutron-rich nuclei impinging on a hydrogen target at ∼250 MeV/nucleon are measured at the RIKEN Radioactive Isotope Beam Factory. Systematically higher cross sections are found for proton removal from nuclei with an even number of protons as compared to odd-proton number projectiles for a given neutron separation energy. Neutron removal cross sections display no even-odd splitting, contrary to nuclear cascade model predictions. Both effects are understood through simple considerations of neutron separation energies and bound state level densities originating in pairing correlations in the daughter nuclei. These conclusions are supported by comparison with semimicroscopic model predictions, highlighting the enhanced role of low-lying level densities in nucleon-removal cross sections from loosely bound nuclei.

The aim of this study was to examine the effects of 2 postweaning feeding management approaches (FEED: 0.8 [HIGH] vs 0.6 [MOD] kg/d target ADG) on the performance of heifers of 2 beef breeds (BREED: Parda de Montaña [PA] vs Pirenaica) calving at 2 yr. Twenty-five heifers previously creep fed before weaning (6 mo) were assigned to 2 planes of nutrition from 6 to 15 mo of age. At 15 mo, they were inseminated, and then received similar diets until weaning of their first calf (4 mo postcalving). Several parameters were measured to analyze growth and development (BW; ADG; size measures at 6 mo, 15 mo, calving, and weaning), performance at puberty and first breeding, and dam and calf performance in the first lactation (calving traits, ADG, milk yield). Metabolic (glucose, cholesterol, NEFA, ß-hydroxybutyrate, and urea) and endocrine status (IGF-I and leptin) were assessed in plasma samples collected every 3 mo from 6 mo to calving and monthly during lactation. No interaction between BREED and FEED was observed. Heifers from the HIGH feeding treatment had higher postweaning ADG than those on the LOW diet. At 15 mo, they had greater BW, heart girth, and external pelvic area, but they did not differ thereafter. All heifers reached puberty at similar BW (55% mature BW) but different ages. Heifers from the HIGH treatment tended (P < 0.09) to be pubertal earlier, and PA heifers were 1.6 mo younger than Pirenaica heifers (P < 0.05) at puberty. At the time of conception (452 ± 59 kg) and calving (471 ± 51 kg), BW was above common recommendations in all groups. Calving traits and performance in lactation did not differ between feeding treatments. BREED only influenced birth weight; PA calves being heavier (P < 0.05), which resulted in a larger calf/cow BW ratio, but no effect on calving difficulty or subsequent performance. Metabolic substrates and hormones depended mostly on sampling date, which was related to current energy and protein intake. Glucose (P < 0.001), cholesterol (P < 0.001), and IGF-I (P < 0.05) were greater during the postweaning phase in heifers on the HIGH diet, and persistent physiological effects were observed during lactation. Age at puberty was negatively related with IGF-I (r = -0.43, P < 0.001), but not with leptin concentrations. In conclusion, regardless of breed, a moderate growth rate ensured adequate heifer development and performance until the first lactation, whereas no advantage was gained from enhanced postweaning gains.

Accelerated growth programs during prepubertal periods have been promoted to advance the first calving of beef heifers. The objectives of the present study were to evaluate nutrition-induced changes on first lactation milk yield and composition and on gene expression of the mammary gland in Parda de Montaña primiparous cows. Female calves ( = 16) were involved in a 2 × 2 factorial experiment. In the preweaning period (PRE-W; 0-6 mo), female calves were either fed a creep feed supplement (Creep) or fed only their dam's milk (Control). In the postweaning period (POST-W; 6-15 mo), heifers received either a high-energy diet (91.7 MJ/d) or a moderate-energy diet (79.3 MJ/d). All the heifers were managed together from breeding (15 mo) to the end of their first lactation (32 mo). Animal performance; milk production and quantity during the first lactation; plasma glucose, IGF-I, and leptin concentrations; and RNA samples from the mammary gland at the end of the first lactation of the primiparous cows (32 mo) were analyzed. The BW and ADG of the primiparous cow during its first lactation were not different among treatments; however, creep feeding during PRE-W reduced milk production ( < 0.01), milk CP, crude fat, lactose, nonfat solids, and casein content throughout lactation and increased somatic cell count in the third ( < 0.05) and fourth month of lactation ( < 0.10). The energy level during the POST-W had no effect on milk production and quality. Gene expression in the mammary gland was affected by the diet in the PRE-W and POST-W, with the PRE-W diet having the greatest impact. During the PRE-W, creep feeding resulted in upregulation of genes related to immune response and chemokine activity, suggesting that these animals might be in a compromised immune status. Therefore, this strategy would not be recommendable; meanwhile, increasing the energy level in the diet during the POST-W would be recommendable, because it had no deleterious effects on milk yield and composition.

Shrubby legumes in Mediterranean-type ecosystems face strong nutrient limitations that worsen in summer, when water is absent. Nitrogen-fixing legumes are likely to be able to switch between soil N and atmospheric N (N2 ) sources to adjust the C costs of N acquisition in different seasons. We investigated the utilisation of different inorganic N sources by two indigenous shrubby legumes (Cytisus multiflorus and Cytisus scoparius). Plant performance in terms of photosynthesis and biomass production was also analysed. Plants were cultivated in sterile river sand supplied with Hoagland nutrient solution, grown in N-free nutrient solution and inoculated with effective rhizobial strains from nodules of adult plants of the same species. A second treatment consisted of plants given 500 µm NH4 NO3 added into the nutrient solution. In a third treatment, plants were watered with another source of N (500 µm NH4 NO3 ) as well as being inoculated with effective rhizobial strains. The application of NH4 NO3 to the legumes resulted in a larger increase in plant dry matter. Carbon construction costs were higher in plants supplied with mineral and symbiotic N sources and always higher in the endemic C. multiflorus. Differences in photosynthesis rates were only observed between species, regardless of the N source. Non-fertilised inoculated plants had more effective root nodules and a clear dependence on N2 fixation. We propose that the ability of C. scoparius to change N source makes it a plastic species, which would account for its broader distribution in nature.

BACKGROUND: Conflicting results have been recently reported for the accuracy of the Endoscopic Reference Score (EREFS), an standardised endoscopic classification, to predict the histological activity of eosinophilic oesophagitis (EoE). AIM: To evaluate the accuracy of the EREFS to predict either histological or clinical activity of EoE. METHODS: Prospective multicentre study conducted in eight Spanish centres evaluating adult EoE patients, either naïve or after treatment. Symptoms were evaluated before upper endoscopy through the Dysphagia Symptom Score, whereas researchers scored the EREFS immediately after the endoscopic procedure, unaware of the histological outcome. RESULTS: One hundred and forty-five EoE patients undergoing 240 consecutive endoscopic procedures were included. Exudates (P = 0.03), furrows (P = 0.03) and a composite score of inflammatory signs (exudates, furrows and oedema) (P < 0.001) accurately predicted histological activity. Exudates were the only endoscopic sign showing a good correlation with histological outcome after therapy. Furrows and oedema persisted in 50% and 70% of patients despite histological remission. No endoscopic feature exceeded 70% accuracy to predict histological activity. Likewise, no endoscopic finding could adequately predict dysphagia severity. Crepe paper mucosa, diffuse exudates and severe rings correlated with higher symptom scores. CONCLUSIONS: Endoscopic findings assessed by the Endoscopic Reference Score did not correlate with histological or clinical disease activity in adult EoE patients. Only exudates correlated with peak eosinophil count and histological outcome, whereas furrows and oedema persisted in over half of patients despite histological remission.

BACKGROUND AND OBJECTIVE: Eosinophilic esophagitis (EoE) is an increasingly prevalent chronic inflammatory disease of the esophagus with an immunoallergic etiology. Few studies have been published on EoE in children and adolescents. The objective of this study was to analyze the demographic, clinical, serologic, endoscopic-histologic, and atopic characteristics of pediatric patients with EoE and to identify atopic and digestive comorbidities. METHODS: We conducted a prospective observational study in children and adolescents (<16 years) assessed in a specialized multidisciplinary EoE unit in a tertiary referral hospital in a central region of Spain between 2011 and 2015. RESULTS: Thirty-five patients were included in the study. Twenty-eight (80%) were male. The mean age was 9.6 years, 83% were atopic, and 28% reported a family history of atopy. The most common symptom was dysphagia (51%). Eosinophilia was detected in the blood of 60% of patients. Eosinophil cationic protein and total IgE were elevated in 88% and 77% of patients, respectively. The most frequent endoscopic finding was linear grooves (57%). Skin tests with aeroallergens were positive in 82% of patients (pollen 62% and food 60%). The main atopic comorbidities were asthma (48%) and rhinoconjunctivitis (37%). Digestive diseases were more often associated with gastritis and Helicobacter pylori infection (17%). CONCLUSIONS: Our results are similar to those previously reported. EoE is more common in boys and in individuals with a history of atopy and sensitization to airborne allergens and food. These results support the consideration of EoE as an atopic disease and underline the important role of allergists in early diagnosis and treatment.

These results support the consideration of EoE as an atopic disease and underline the important role of allergists in early diagnosis and treatment (AU)


Introducción y objetivos: La esofagitis eosinofílica (EoE) es una enfermedad inflamatoria crónica, emergente del esófago, con etiología inmunoalérgica. Hasta ahora, pocos estudios sobre esta enfermedad en niños y adolescentes han sido publicados. El objetivo de este estudio era estudiar las características epidemiológicas, clínicas, serológicas y endoscópico-histológicas de estos pacientes y las comorbilidades alergológicas y digestivas. Metodología: Es un estudio observacional descriptivo en niños y adolescentes (<16 años), estudiados en una unidad multidisciplinar en un hospital de tercer nivel en una región del centro de España. Resultados: Se incluyeron en el estudio 35 pacientes. 28 eran hombres (80%). La edad media era 9,6 años, el 83% eran atópicos y tenían antecedentes familiares de atopia el 28%. El síntoma más común fue la disfagia (51%). Se detectó eosinofilia en sangre en el 60% de los pacientes. La proteína catiónica del eosinófilo y la Ig E total estaban elevadas en el 78% y 77% de los pacientes, respectivamente. El hallazgo endoscópico más frecuente fueron los surcos lineales (62%). Loa tests cutáneos fueron positivos en el 82% de los pacientes (polen 62%, alimentos 60%). Las comorbilidades atópicas fueron, asma (48%) y rinoconjuntivitis (37%). Las enfermedades digestivas más asociadas fueron gastritis e infección por Helicobacter pylori (17%). Conclusiones: Nuestros resultados son similares a los publicados previamente. La EoE es mas frecuente en niños, con historia de atopia, sensibilizados a aeroalérgenos o alimentos. Estos resultados apoyan que la EoE pueda considerarse una enfermedad atópica y el importante papel del alergólogo en el diagnóstico y tratamiento precoz de esta enfermedad (AU)

RESUMO

Excitation spectra of ^{11}C are measured in the ^{12}C(p,d) reaction near the η^{'} emission threshold. A proton beam extracted from the synchrotron SIS-18 at GSI with an incident energy of 2.5 GeV impinges on a carbon target. The momenta of deuterons emitted at 0° are precisely measured with the fragment separator (FRS) operated as a spectrometer. In contrast to theoretical predictions on the possible existence of deeply bound η^{'}-mesic states in carbon nuclei, no distinct structures are observed associated with the formation of bound states. The spectra are analyzed to set stringent constraints on the formation cross section and on the hitherto barely known η^{'}-nucleus interaction.

BACKGROUND: Proton pump inhibitor-responsive oesophageal eosinophilia (PPI-REE) is common in patients with suspected eosinophilic oesophagitis (EoE). However, the long-term efficacy of PPIs and the best maintenance doses are yet to be defined. AIM: To evaluate the durability of the response to PPI therapy after tapering PPI doses in PPI-REE patients. METHODS: Prospective study conducted on PPI-REE patients. Upon complete remission on high-dose PPI therapy (omeprazole 40 mg b.d. for 8 weeks), PPI doses were tapered followed by an endoscopic procedure after each dose reduction. The primary outcomes were sustained clinical and histological remission (<15 eos/HPF) after decreasing PPI doses. RESULTS: From a total of 121 patients with suspected EoE, 40 (33%) achieved complete remission on high-dose PPIs and were given a diagnosis of PPI-REE. No patient in histological remission showed symptom relapse, but half of patients with relapsing oesophageal inflammation were in clinical remission. After reduction to omeprazole 40 mg once daily, 38/31 (81%) remained in complete remission. Among these latter patients, 15/18 (83%) were kept in remission with omeprazole 20 mg once daily. As for side effects, only asymptomatic hypertransaminasemia and oesophageal candidiasis were observed in two patients while receiving high doses of omeprazole. CONCLUSIONS: Most PPI-responsive oesophageal eosinophilia patients show sustained clinical and histological remission with daily PPI doses equal to or below 40 mg of omeprazole. As adverse effects only appeared with the highest dose of omeprazole, it would be advisable to individualise the dose of PPIs for each patient, lowering it to the minimum capable of maintaining the disease controlled.
